Enhanced feedback modes

Introduce three feedback modes that control what learners see after each question
Full Feedback shows correct or incorrect status, reveals the correct answer and shows the explanation
Basic Feedback shows only whether the answer was correct or incorrect
No Feedback acts as an exam mode and automatically moves to the next question without revealing any details
